Our provider networks consist of Chiropractors, Physical and Occupational Therapists, Speech Language Pathologists, Massage Therapists, Acupuncturists, and Fitness Centers. If you are interested in joining our network, please complete the appropriate request form.

CAQH

Palladian requires all practitioners to credential through the “Universal Provider DataSource” (UPD), a service of the Council for Affordable Quality Healthcare (CAQH). This service has become the industry standard for collecting provider data used in credentialing. The CAQH process allows providers to complete one application which can be used universally by all participating health plans and healthcare organizations. Provider Information

As a participating provider, it is very important that the information we have on record is kept current. Any changes in your credentialing information should be updated with the Universal Provider DataSource. Additionally, please submit Provider Change forms to Palladian for timely updates of contact numbers and email addresses.
